2. Solicitation number RFP-19-PHX-04 is being issued as a Request for Proposal (RFP).

4. This procurement will be issued on an unrestricted basis. North American Industry Classification System (NAICS) code is 923120 Administration of Public Health Programs, and the applicable size standard is not established.
5. This requirement will consists of five (5) line items: 0001 - Tribal Health Steering Committee (THSC) 0002 - Option Year One (1): Tribal Health Steering Committee (THSC) 0003 - Option Year Two (2): Tribal Health Steering Committee (THSC) 0004 - Option Year Three (3): Tribal Health Steering Committee (THSC) 0005 - Option Year Four (4): Tribal Health Steering Committee (THSC)
6. This requirement is to support and assist Indian Health Service with consultation and coordination of Indian tribes on a government-to-government basis to address issues concerning Indian tribal self-government, tribal health issues, programs, mental health, health statistics, immunization services and public health-related issues for Phoenix Area Indian Health Service (PAIHS), located in Phoenix, Arizona. See attach scope of work.
The Government reserves the right to issue a Firm-Fixed Priced. Proposal shall be submitted via email with subject identifying the RFP number to be considered. This requirement contract will be funded at the time of order.
Best value will be determined by meeting technical and past performance and lowest cost alternative to the government that provide the greatest overall benefit in response to this requirement. Proposals are all inclusive and shall be submitted on via email to be considered. Final price shall include any discounts for the items listed. IHS Phoenix Area intends to solicit, negotiate, and award a requirement contract inclusive of all line items, the Government reserves the right to issue more than one award. Price will not be evaluated until Technical Capabilities and Past Performance is rated acceptable.
7. The location and address of the service unit location is provided in the scope of work. The period of performance will be 12 months with four (4), 12-month option years is anticipated to be made as a result of this solicitation. The total length of contract will not exceed 5 years from service.

10. The Defense Priorities and Allocations System (DPAS) is not Applicable to this acquisition.
11. RESPONSES ARE DUE BY: Monday, September 30, 2019 by 2:00 p.m. Arizona Standard Time (NOTE Arizona does not observe Daylight Savings Time). Electronic submissions of offers are preferred and can be sent via E-mail to [email protected]. The Subject in the email must contain the RFP number, example; "RFP-19-PHX-04 - (company Name)". The size limit for incoming E-mail is 10MB.
